Southwest EAP offers comprehensive in-home assessments and care planning for elderly relatives. This includes an in-home health assessment, mental status examination, activities of daily living evaluation and consultation with the family to determine the appropriate level of care needed to ensure safety and peace of mind.

  • In-home assessments - This assessment provides recommendations that allow us to develop a detailed care plan. Individuals are assessed and evaluated in multiple areas including safety management, health management, community participation, healthy eating, household care needs, and psychosocial adjustment to provide a comprehensive plan of care.

  • Assisted referrals to community resources and facilities - This may include coordinating and monitoring home health services, household maintenance and repairs, lawn care, meals, deliveries and transportation. It may also involve referral to assisted living facilities or skilled care facilities.
